While braces are often the stars for straightening smiles, they're not the only cast members in an orthodontic production. Orthodontists occasionally turn to other appliances if the bite problem is challenging. Whatever the tool, though, they usually have something in common—they use the principle of anchorage.
To understand anchorage, let's first consider the classic kid's game Tug of War. With teams on either end of a rope, the object is to pull the opposing team across the center line before they pull you. To maximize your pulling force, the player at the back of your rope, usually your stoutest member, holds steady or "anchors" the rest of the team.
Like a Tug of War team, braces exert force against the teeth. This stimulates the supporting periodontal ligament to remodel itself and allow the teeth to move. The braces use the teeth they are attached to as anchors, which in a lot of cases are the back teeth. By attaching a thin wire to the brackets or braces on the teeth, the orthodontist includes all the teeth on the arch, from one end to the other. Anchored in place, the wire can maintain a constant pressure against the teeth to move them.
But not all bite situations are this straightforward. Sometimes an orthodontist needs to influence jaw growth in addition to teeth movement. For this purpose, they often use orthodontic headgear, which runs around the back of the head or neck and attaches to orthodontic brackets on the teeth. It still involves an anchor but in this case it's the patient's own skull.
In some situations, an orthodontist may feel he or she needs more anchorage as the teeth alone may not be enough. For this, they might establish a separate or additional anchor point using a temporary anchorage device (TAD). A TAD resembles a tiny screw that's inserted into the jawbone near the tooth intended for movement. The orthodontist can then attach the TAD to braces hardware using some form of elastics. After treatment, they remove the TAD.
These are just a couple examples of specialized tools an orthodontist can use for bite correction. Thanks to them and similar devices, even the most complex bite problem can be overcome to create a healthier and more attractive smile.

If you have chronic jaw pain, you may be one of an estimated 10 million Americans suffering from temporomandibular joint disorders (TMD). If so, it's quite possible you're also coping with other health conditions.
TMD is an umbrella term for disorders affecting the temporomandibular (jaw) joints, muscles and adjoining tissues. The most common symptoms are limited jaw function and severe pain. Determining the causes for these disorders can be difficult, but trauma, bite or dental problems, stress and teeth clenching habits seem to be the top factors. Women of childbearing age are most susceptible to these disorders.
In recent years we've also learned that many people with TMD also experience other conditions. In a recent survey of TMD patients, two-thirds reported having three or more other health conditions, the most frequent being fibromyalgia, chronic fatigue syndrome, rheumatoid arthritis or chronic headaches. Researchers are actively exploring if any systemic connections exist between TMD and these other conditions, and how these connections might affect treatment changes and advances for all of them including TMD.
In the meantime, there remain two basic approaches for treating TMD symptoms. The most aggressive and invasive approach is to surgically correct perceived defects in the jaw structure. Unfortunately, the results from this approach have been mixed in their effectiveness, with some patients even reporting worse symptoms afterward.
The more conservative approach is to treat TMD orthopedically, like other joint problems. These less invasive techniques include the use of moist heat or ice to reduce swelling, physical therapy and medication to relieve pain or reduce muscle spasming. Patients are also encouraged to adopt softer diets with foods that are easier to chew. And dentists can also provide custom-fitted bite guards to help ease the stress on the joints and muscles as well as reduce any teeth grinding habits.
As we learn more about TMD and its relationship to other health conditions, we hope to improve diagnosis and treatment. Until then, most dentists and physicians recommend TMD patients try the more conservative treatments first, and only consider surgery if this proves unsatisfactory. It may take some trial and error, but there are ways now to ease the discomfort of TMD.

It’s July—and that means it’s National Park and Recreation Month! If you’re like a lot of families, you might already be planning a trip to one of the nation’s 58 national parks, or one of the thousands of state outdoor recreational areas across the country.
Temporarily escaping the stresses of daily life in the great outdoors is a wonderful way to refresh both the soul and the body. But that’s not an excuse to neglect all your responsibilities. That includes making provisions to care for your teeth while you’re away from home—you are bringing them with you, aren’t you?
Here are three ways you can take care of your teeth during your outdoor getaway.
Keep up your daily hygiene. While you’re packing extra socks, granola and moleskin, be sure to include your toothbrush, toothpaste and floss. Just a few days of neglecting your regular oral hygiene can give bacterial plaque a chance to build up. You could even come back from your trip with the beginnings of gingivitis, an early form of gum disease. If you’re trying to pack light, take along travel-size toothpaste tubes or pre-threaded floss picks to make it easier.
Eat dental-friendly snacks and food. Escaping your usual dietary choices doesn’t mean you should take a vacation from good nutrition. Whether you’re in camp or on the trail, eat whole fruits, grains or cheeses, and avoid snacks and foods with added sugar that feeds disease-causing bacteria in the mouth. The same goes for beverages—keep your intake of sodas and sports or energy drinks (all loaded with added sugar and acid) to a bare minimum. Instead, hydrate with water.
Be prepared for emergencies. Exploration through hiking, canoeing and other physical activities is a great part of the outdoor park experience. But it also increases your risk of injury, especially in rough terrain. Before you head out, take some time to research medical and dental resources near your vacation destination in case you or a family member will need immediate care. Having that information handy can save time in the event of an emergency.
An outdoor park trip can be the experience of a lifetime. Just be sure to follow these simple tips to care for and protect your teeth. Doing so will help ensure that your memories of this summer’s outing will be pleasant ones.
